In Person Appointments
If this is your initial in-person appointment, please arrive 15 minutes
early and bring your insurance card.
Important Information
When you arrive, your paperwork will be sitting on a clip board at the front desk with your name and your appointment time. If you are unable to arrive to your initial appointment early, you may print and fill out paperwork prior to your appointment. Initial Paperwork is in PDF link below. If the client is an adolescent under the age of 18, then a parent or guardian will need to be present for the first appointment and will sign off on all Paperwork. Please Note: If you choose not to arrive 15 minutes early or complete the paperwork ahead of time, your paperwork will cut into your 40-minute time slot. Appointments cannot be pushed back due to late arrival or paperwork. If you are late to your appointment more than 20 minutes after your start time, your appointment will be considered a no-show.
