Clients are responsible for knowing if their Cornerstone Counselor is covered by their insurance network. You can check by Calling the member services number on the back of your insurance card. Select the benefits option. When you get a representative on the phone, ask them for: Outpatient Mental Health Professional benefits. Then say,…I have a tax id = 391746222. This should inform them of their deductible, coinsurance, copays. You can also go to https://www.cornerstonecounseling.com/directory for more information. It is the client’s responsibility to bring their insurance card during the first appointment If you do not bring your insurance card you will be billed as self-pay. If you forgot to bring your insurance card during the first appointment, you will have 24 hours to email a picture of your insurance card to ccshelp@ccs2.com. The Client is responsible for all charges regardless of insurance coverage. It is the clinic’s policy that the portion of the fee not covered by insurance be paid at the time of services are rendered. All clients are responsible to provide clinic with most updated insurance information and to contact us should insurance coverage be changed. If clients choose not to go through insurance, do not have insurance or would like to pay privately Kara’s Private Pay rates are the following and nonnegotiable. To better accommodate self-paying clients, rates have been modified to a reduced rate to compare to what insurance providers pay. Self Pay is due at time of service.

45-minute session $125.00
60-minute session $150.00

Due to the high waitlist, client will be charged for missed appointment unless canceled 24 hours in advanced. Our policy is to charge for missed appointments at the rate of a normal office visit, however Kara understands that things come up. Below is the modified rates Kara uses. Please note you will be billed directly for missed appointments. Insurance carriers do not assume financial responsibility for missed appointments and charges will be a out of pocket expense. If there are 4 cancelations or 3 no shows the client will automatically be discharged as a client.
